Question Four Moment - etymology

#1
Forgive my J5 virginity. I've come across the 'question 4 moment' phrase often enough, and understand it to mean 'what has changed?'

But is it a formal part of mil planning? Doesn't seem to tie in with q4 of the combat estimate process, so which planning process does this equate to?

Just curious / a sad ******* doctrine geek.
