Insight needed...

I am currently drafting an essay with regards to organisational learning and knowledge management within the military.

I am hoping to look at any implementations of learning methods (such as the US military produced After-Action Reports). Are there any standard methods of "formalised" de-briefs or reviews and also are there any reliable pathways for soldier idea implementation to be passed "up the chain" so to speak, or any other recognised procedures for anything to do with bettering current practices? (This can relate to anything such as the design of equipment, necessary equipment, new working methods etc.)

I am doing parallel research upon this, but thought I may as well speak to those who have served/are already serving for a first-hand insight.
