Perhaps by 1917/18 after conscription was introduced and they became desperate for manpower he was medically examined and judged fit for service, but in a limited way?

Couldn't he have just walked into a different recruiting office? I can't see it likely that anyone would have checked that he had previously been medically discharged, not with manual record keeping. Then it's just a matter of bluffing his way through a medical that might have been less onerous for a non-infantry role.

Just a thought, can the OP or anyone else make out the signatory or any of the writing on the bottom of the certificate. It might lead in a new direction ?
Perhaps a scan of the document rather than a photo would make it clearer ?
