"Certificate of Entitlement" from the BFPO - VAT refund.

Up until now, Amazon refunded VAT without any certificates of a BFPO address. However, under UK VAT Legislation, a "Certificate of Entitlement" for BFPO addresses is now required before a refund can take place. This certificate confirms that the items have been shipped to and received by a member of the BFPO.

How can I obtain this certificate?

Thanks in advance.
Just adapted an online version of the BFPO 1510 form to use with Amazon.

I'd recommend saving the file and opening it in word/open office etc as google docs doesn't show correct formatting of the tables.

Hopefully this should help.
