It is the same as the additional info that is on the 9-liner, but it is a separate thing. It would be used to give vital casualty info to a medical team/next in casevac chain when handing over a casualty.

MSR et al, cheers, this is a good resource.
I am too much of an IT-mong to resize to A6 and print properly
but they are easy enough to copy out onto waterproof notepaper in perma pen that will not run, then Nirex/fablon up. It still works
Something that I found useful (on ex and on Cambrian, not done it for real!) was having a Call For Fire cheatsheet made up and ready to go. All the headings of what you need to give over the radio for a Fire Mission on one side, then on the other side a table of these details (Grid, Dir, etc etc) for each tgt - so (I imagine) you could call for fire again onto any number of previously engaged targets.
Also, there is a cheatsheet from the Mine Information Training Centre that is handy, details all the steps to be taken i.e. LFP, MAM etc, and includes a mine data report format on the reverse.
When deciding which Course of Action to take, before you fill out your effects schematic, it's a fair idea if you have a table filled out. The 5 principles of attacks along the top, 3 different COAs down the side. The one that gets the most ticks in the boxes for fulfilling each principle, is the COA that should get chosen...
3 Warning Order cards is also a good idea, you need 1 for your initial warning order, 1 for your full one, and 1 to copy down the warning order you received off your comd. Plus they are forever going missing and getting hoarded by the runner
Quick question on the CASEVAC format - it says for use with US & NATO helos - is this '9 liner' format used by UK forces too? I'm sure its laid out in the TAM but I haven't got mine to hand at the mo... So a heads-up on whether this CASEVAC is universal or just when working with other nations would be grand.

To print them A4 just print 1 per page, for A5 two per page and A6 4 per page
