Which medal is this?‘Good God!’ he laughed, and slowly filled his pipe,

Royal Red Cross 1st Class (RRC) and Royal Red Cross 2nd Class (ARRC)
The Royal Red Cross
The Royal Red Cross is only awarded to members of the Nursing Services. It consists of two classes. Recipients of the Royal Red Cross 1st Class are known as Members, while recipients of the 2nd Class are known as Associates.
The Royal Red Cross is awarded "for exceptional devotion and competency in the performance of actual nursing duties."
The ARRC is awarded "for�special devotion and competency in the performance of actual nursing duties."

I believe it is the Royal Red Cross (2nd Class)
This decoration has the distinction of being confined to females until 1976. It is conferred on members of the nursing services regardless of rank. A second class award was introduced in 1917. Holders of the 2nd class are promoted to 1st class on second awards. Holders of the 1st Class decoration are known as Members (MRRC) while recipients of the 2nd Class are Associates (ARRC)From the Utmost End of the Earth
