OK - I went to your questionnaire and filled it in as best I could.

I work for a multinational company with 100,000 employees. It was difficult to put sensible answers in - for example your radio button for company numbers only went up to 250+. Similarly, my company operates world wide - not an available option for that question.

Methinks a bit of questionnaire redesign is called for.
